Protected health information obtained by Wellstart will be recorded in your record and used to provide you with necessary medical supplies. Wellstart will also use your health information for payment and reimbursement. A bill may be sent to you or a third-party payer and the information on or accompanying the bill may include information that identifies you, as well as your diagnosis and supplies used. Wellstart will also use your health information for regular healthcare operations. Teammates of Wellstart’s quality improvement team may use information in your health record to assess the Company’s compliance with applicable law, regulation and industry standards.
